Office Hours
  • Mondays to Fridays we are open from 9:00am to 6:00pm
  • We are close for lunch break at 12:00pm to 1:00pm
  • Saturdays we are open from 9:00am to 1:00pm
Prescription Refills
  • If you are down to your last 5-day supply of medications please contact us immediately, this will give both parties sufficient time to address your medication refill needs. We prefer that you ask your pharmacy to fax a request to us.
  • Do not wait till you are down to your last pill to contact us.
Hospital Affiliations
  • Dr. Roque is on staff at the Swedish Covenant Hospital in Chicago and the St. Francis Hospital in Evanston.
